The Senegal international had been coveted by a succession of Chelsea managers in the past but eventually made the move to the Premier League in the summer to fill the void left by Andreas Christensen and Antonio Rudiger.
Koulibaly enjoyed an encouraging start under Thomas Tuchel but suffered a chastening experience in a 3-0 defeat during which he was sent off for two bookable offences.
The 31-year-old has struggled to hold down a place since the appointment of Graham Potter with an injury rendering him unavailable for the last three matches.
