He was then found unresponsive at HMP Forest Bank, a Category B jail in Salford, Greater Manchester. Giving evidence at the inquest, pathologist Dr Naomi Carter said she was unable to answer whether Mr McDonagh would have died if he had not been prescribed methadone. She did, though, say the man died from bronchopneumonia after a "depressant" of his nervous system, which was likely caused by a "combination" of medications, Manchester Evening News reports. Speaking at the hearing, Ms Pomeroy said: "I physically ache not being able to see him, touch him, smell him, or hear him. I feel entirely heartbroken. I have had to endure a long four-year wait to get to this point." She told the court she "could not understand" how her son died whilst in the care of "those supposed to be taking care of him." "I don't want any other family to go through what we have experienced," she added. "I'm grateful every day for the time we got to share with him."
